How do I complete the survey? Read the statement, and choose how strongly you agree or disagree with the statement. The green area indicates that you agree and the red are indicates that you disagree. Once you have decided, you must click on the appropriate button. If you are having difficulty in deciding, select the one you think most closely describes your experience. If you did not make use of a particular service you can select the NA (Not Applicable) button or choose not to select a button at all. How do I know my answers have been saved? Once you have successfully completed a survey and your survey has been saved, you will be re-directed to a Thank You page. If you reach this page then your survey has been successfully processed. What if an error message appears or the system freezes? If an error message appears, click on the Back button to get back to your questionnaire and resubmit. If you exit, you will lose the answers you captured. If the system freezes at any stage, click on the refresh button and start again.
